Performance Art Submission Checklist
Request for Application Assistance
Applicants will need to download at least one checklist and the “Terms of Application.” All applicants will need to print 2 “Terms of application” and sign both to complete a application and make two copies of their app check list and CDs.DVDs.
After you have completed your application(s) please send two copies of your checklist and the 2 signed copies of terms of application with two CDs or DVDs containing all required documents (and for PA apps you need to include duplicate copies of work samples in addition)
